During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced monitoring inspection was initiated on November 21, 2024 and completed on December 5, 2024 in response to a self-report received by the licensing office on November 21, 2024 relating to supervision of children. The inspector reviewed one child's record and three staff records on-site. The inspection included document review, tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, and protection, and guidance. On 11/20/2024, Staff #1 exit the classroom with two children to go outside; leaving Child #1 unsupervised for two (2) minutes before the staff realized Child #1 was not with the group.
Plan of Correction: We have retrained all staff and established additional transitional procedures. Training was conducted by the Educational Director and the Principal. The employee was placed on administrative leave until the investigation was completed.

Children under 10 years of age shall always be within actual sight and sound supervision of staff. On 11/20/2024, a three year old child was unsupervised for approximately two minutes inside the classroom.
