During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on April 14, 2025. The on-site inspection began at 1:50pm and ended at 4:55pm. The inspector reviewed compliance in the areas listed above. There were 49 children present and 17 staff. The inspector reviewed six children?s records and six staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The center did not request a search of the child abuse and neglect registry from one state prior to the date of hire for Staff #3 and Staff #5.
Plan of Correction: The request for out-of-state child abuse and neglect was submitted on 3/14/25. We will ensure all background searches and requests are submitted prior to the day of hire.
Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child #1 and Child #5 did not have additional immunizations documented in their records.
Plan of Correction: We have contacted the parents requesting an updated immunization records. We will pull the monthly immunization report that and notify parents that an updated record is needed.
Standard 8VAC20-780-240-B
Staff shall complete orientation training prior to the staff member working along with children and no later than seven days of the date of assuming job responsibilities. Staff #3, who had been employed for one month, did not have documentation of orientation training. Staff #6, who had been employed for two months, did not have documentation of orientation training.
Plan of Correction: The orientation was completed during the first week, but was not signed or dated. We will have 2 people check the new employee checklist to ensure all orientation papers are completed.
Standard 8VAC20-780-370-5
Infants, who cannot turn themselves over and are awake, shall be placed on their stomachs a total of 30 minutes each day. There was no documentation for tummy-time for the infants who were present during the inspection.
Plan of Correction: I was not aware we had to document tummy-time, but we will train our staff who work with infants on documenting the tummy-time each day.
