When staff are supervising children, they are required to always ensure their care, protection, and guidance.
On 11/12/2025, from approximately 5:20pm-5:25pm, Child 1 was left alone inside of the bathroom located in the Preschool 1 classroom. Child 1's teachers had left the classroom for the day during this timeframe. Child 1 was supposed to be transitioned over the the Preschool 2 classroom prior to Staff 1 and Staff 2 leaving for the day. Two children from Preschool 1 were transitioned over to Preschool 2, but Child 1 was inside of the bathroom during the transition and was not transitioned with the rest of the children. Staff did not ensure the care and protection of Child 1.
On 11/12/2025, from approximately 5:20pm-5:25pm, Child 1 was left alone inside of the bathroom located in the Preschool 1 classroom. Child 1's teachers had left the classroom for the day during this timeframe. Child 1 was supposed to be transitioned over the the Preschool 2 classroom prior to Staff 1 and Staff 2 leaving for the day. Two children from Preschool 1 were transitioned over to Preschool 2, but Child 1 was inside of the bathroom during the transition and was not transitioned with the rest of the children. Staff did not ensure the care and protection of Child 1.
Plan of Correction: Staff 1 was released from her job at Goddard. Staff 2 was written up and re-trained on staff supervision and name to face checks and threshold counts.

Staff 2 did not conduct a face to name check as the 2 children walked through the connecting door in between the classrooms from the Preschool 1 class to the Preschool 2 class. After the 2 children transitioned over, Staff 1 and Staff 2 left as their shift was over. At approximately 5:25pm, Staff 3 was checking the trash in the Preschool 1 classroom as part of closing duties and heard Child 1 calling out for help from the restroom located in Preschool 1. Staff 3 found Child 1 in the bathroom and stayed in the classroom until another teacher was able to come to assist Child 1.

The operational responsibilities of the licensee include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.
The facility did not follow their "Threshold Counts and Name-to-Face Checks Policy" on 11/21/2025 when transitioning the Preschool 1 children to Preschool 2 at the end of the day. The Policy states: "As children leave the classroom with their teacher(s), threshold counts and name-to-face checks are conducted to ensure that all children are accounted for with the
group. Threshold Count: This is the practice of a teacher counting the number of children in line and matching that number to the current number in attendance. Threshold counts occur at every doorway, stairwell, or threshold along the way to your final destination.
Name-to-Face Checks: Once the class has reached the final destination, a name-to-face check occurs. The teacher matches the name on the attendance record with the face of the
child as he/she crosses the final threshold. A system for the name-to-face checks is recorded or evident via tablet transitions, paper attendance, or other system in place.
A verbal confirmation of the name-to-face checks occurs (i.e. if transferring children in a tablet, say names aloud as well)."
On 11/21/2025, Child 1 was left alone inside the bathroom located in the Preschool 1 classroom from approximately 5:20-5:25pm. There was no verbal confirmation of the name-to-face check occurring at the time of the incident, nor was there accurate documentation reflecting where Child 1 was when the Preschool 1 children transitioned to the Preschool 2 classroom at the end of the day. Staff did not follow the center's own policies and procedures.
