Inspector Notes

An unannounced monitoring inspection was initiated on 08/25/2024 and completed on 10/16/2024 in response to a self-report received by the licensing office on 08/25/2024 relating to Programs, Staffing and Supervision. The inspector reviewed one staff records on-site. This inspection included document review, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/23/2024. A POC submitted after this date will not appear on the public website.

Behavior guidance shall be constructive in nature, age and stage appropriate and shall be intended to redirect children to appropriate behavior.

On 08/20/2024, Child #1 was repeatedly climbing on shelves when Staff #1 used their knee to push Child #1 off a shelf. Staff #1 attempted to remove Child #1 from a shelf then Child #1 was observed holding their arm, running away from Staff #1 and crying. A red mark was immediately observed on Child #1?s arm by Staff #2.
Plan of Correction: Admin was notified of possible incident. Video footage was reviewed and staff in question was removed from classroom immediately. Child was comforted and needs were attended to. Staff in question was immediately terminated, due to violation of center policy and removed from the building. Remaining staff were counseled on age-appropriate behavior and addressing stress in the childcare setting. CPS, parents and licensing were notified of the incident.
