During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site inspection was initiated on April 25, 2025 completed on April 25, 2025 in response to a complaint received by the licensing office on April 22, 2025 relating to allegations of Physical Plant and Staffing and Supervision. The inspector reviewed one child?s record on-site. This inspection included document review, tour of the facility, interviews and observation.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Though not a part of the original complaint, violations that were not reported were found related to Special Care Provisions and Emergencies and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on May 7, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-400-C
When time out is used as a behavior guidance technique, it shall not be used with infants and toddlers.
Time out was used as a regular behavior guidance technique for more than two months for Child #1 who is a toddler.
Plan of Correction: Staff received retraining that ?thinking time? cannot be used with children in the toddler classroom. Additionally training is being provided to staff for behavior management and classroom guidance.
Standard 8VAC20-780-500-B
The diapering surface shall be used only for diapering or cleaning children.
Staff in Classroom #1 were using the changing table to prepare food. There was a storage container on the changing table in Classroom #2.
