During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site inspection was initiated on March 9, 2026 and completed on March 12, 2026 in response to a complaint received by the licensing office on March 4, 2026 relating to allegations of Administration and Programs. The inspector reviewed one child?s records on-site. This inspection included document review and interviews.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on March 23, 2026. A POC submitted after this date will not appear on the public website.

The operational responsibilities of the license shall include ensuring that the center?s activities, services, and facilities are maintained in compliance with the center?s own policies and procedures that are required.
The center?s termination policies states ?The center director reserves the right to terminate any family, at any time, from the program that is deemed disruptive. As best as possible and prior to this decision, there will be open communication between the child?s guardian, teachers and leadership within the center.?
On March 4, 2026, Child #1?s parents were informed that the child?s enrollment was terminated effective immediately without warning or reason. The program confirmed that the family was not disruptive and there was no open communication regarding the decision prior to the decision
Plan of Correction: Child was never terminated from care. During a phone call, director told parents to pick child up ASAP due to disruptive & unsafe behavior inside the classroom.

Parents shall be informed of reasons for termination of services.
The center abruptly terminated childcare services for Child #1 on March 4, 2026. The reasons for termination of services were not provided to the parents.

Children?s records shall be available to parents on request.
The parent of Child #1 requested records by email on March 4, 2026. The records were not provided.
Plan of Correction: Child?s records were provided via Brightwheel, in person and by email to the family. Family refused to receive them in person on 3/4/26. Use of Brightwheel will be mandatory for families moving forward, due to communication and safety.
