During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site (monitoring/renewal/focused) inspection was initiated and completed on November 5, 2025. The on-site inspection began at 11:50 am and ended at 1:20 pm. The inspector reviewed compliance in the areas listed above. There were sixteen children present and four staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Staff #4 had a national criminal background check completed one day after beginning employment.
Plan of Correction: In the future, a staff will not begin employment until after a national criminal records check is completed.
Standard 8VAC20-780-160-A
The center is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children. Staff #4 has been employed for a week and there is not a TB screening.
Plan of Correction: Staff will have to provide TB screening results before returning to work.
