During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the program was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 4/8/25. The on-site inspection began at 3:15pm and ended at 4:10pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers. The inspector reviewed 1 child's record and 2 caregiver records while on-site. This inspection included document review, tour of the daycare areas of the home and interview. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/16/25. A POC submitted after this date will not appear on the public website.

Prior to employment, all caregivers shall have completed a national fingerprint search (CRC-criminal record check).

1. There was no documentation of a completed CRC for assistant caregiver #1 who began working in October 2024 and reportedly continued to provide direct-care to children as recent as last week.
2. There was no documentation of a completed CRC for assistant caregiver #2 who was observed providing direct-care to children today and reportedly began employment this week.

The provider shall obtain from each caregiver at the time of hire prior to coming into contact with children a current Report of tuberculosis (TB) screening documenting the absence of tuberculosis in a communicable form.
