Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 8/12/2024 and completed on 8/12/2024, as a part of the conditional licensure period. The on-site inspection began at 12:00 pm and ended at 2:10 pm. The inspector reviewed compliance in the areas listed above. CDC: There were 11 children present and 9 classroom staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included

Based on observation the center failed to ensure that ground supports shall be covered with materials that protect children from injury.
Evidence:
The ground supports for the climbing apparatus have concrete that is exposed and could injure a child if they fell on it.
Plan of Correction: The director moved around the mulch to cover the ground support and will request a covering be placed over the concrete area.
