Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 12/3/25 and completed on 12/17/25 . The on-site inspection began at 1:40 pm and ended at 2:50 pm. The inspector reviewed compliance in the areas listed above. There were 45 children present and 13 staff. The inspector reviewed five children?s records and five staff records on-site

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/31/25. A POC submitted after this date will not appear on the public website.
Violations
6Staff #1had been employed one month and staff #3 had been employed two months did not have documentation of a completed sworn statement of affirmation.
Staff #1 had no documentation and had been employed for one month, Staff #2 had no documentation and had been employed seven months, Staff #3 had no documentation and had been employed two months, Staff #4 had no documentation and had been employed three months, and Staff #5 had no documentation and had been employed three months.
The concrete footing under the yellow piece of play equipment had ground supports exposed.
The center did not have a battery operated flashlight.
The snacks being served to children only had one component.
Staff #1, employed wo months and Staff #3 employed three months do not have documentation of reference checks nor, documentation of diploma or transcripts.
