An unannounced, renewal inspection was completed on December 20, 2024.The inspection began at 8:55 a.m. and ended at 10:05 a.m. The inspector reviewed compliance in the areas listed above. There were 7 children present and 2 caregivers; the point total was 15. The inspector
reviewed 4 children?s records and 2 caregiver records. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on

Repeat Violation: The provider shall maintain an up-to-date record at the family day home for each enrolled child.

The provider did not have a record for child D that has been attending the program for 1 day.

Repeat Violation: The provider shall obtain the parent's written acknowledgement of the receipt of certain required information.

Child A, child B, and child C's record was missing written acknowledgement of the receipt of the policies for the administration of medications, whether or not the family day home has liability insurance, whether or not the family day home administers prescription medication, provision of the emergency preparedness and response plan, and information for parents.

Repeat Violation: The provider shall ensure that emergency evacuation procedures are practiced monthly with all caregivers and children in care during all shifts that children are in care.
