An unannounced inspection was initiated on 01/08/2025 and completed on 01/13/2025 in response to a complaint received by the licensing office on 12/20/2024 relating to allegations of Religious Exempt and Background Check codes .The inspector reviewed 5 children?s records and 6 staff records on-site 01/08/2025 and off-site on 01/09/2025.This inspection included document review , tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

The center must obtain a completed national criminal background check (fingerprinting) prior to the employee's first day of employment. Staff #1, Staff #2, Staff #3 , Staff #4 , Staff #5 and Staff #6 did not have a completed national criminal history check that is processes by the Virginia State Police and the FBI through the designated fingerprint vendor called Fieldprint.
Staff #1 has been employed for about four months, Staff #2, Staff #3, Staff #5 have been employed for at least 6 months. Staff #4 has been employed for 2 months. Staff #6 has been employed for 3 months.
Plan of Correction: All staff had a Sworn statement, Central Registry and a background check that covered National , state and county criminal, employment, education , VR and sex offender registry this was completed by a third party agency.

Once I was notified the criminal background check needed to be done by Fieldprint. I immediately had all staff scheduled for fingerprints and they were completed by 12/28/2024.

Going forward all National criminal background checks will be completed through Fieldprint.
