Inspector Notes

An unannounced, on-site renewal inspection was initiated on 02/12/2025 and completed on 02/12/2025. The on-site inspection began at 9:45 am and ended at 11:15 am. The inspector reviewed compliance in the areas listed above. There were 25 children present and 3 staff. The inspector reviewed 5 children's records and 4 staff records on-site on 02/12/2025.

Standard 8VAC20-780-160-A
Documentation of the required tuberculosis screening shall have been completed within the last 30 calendar days of the date of employment and prior to coming into contact with children.

The tuberculosis screening for staff 2 was completed 4 months prior to employment.
Plan of Correction: The program director stated that, going forward, the required TB screening will be obtained within 30 days prior to employment.
Standard 8VAC20-780-240-A
The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.

Staff 1, who has been employed more than 90 days, has not completed the Virginia Department of Education-sponsored orientation course.
Plan of Correction: The program director stated that staff 1 is working on this training and will have completed soon.
Standard 8VAC20-780-540-E
The following nonmedical emergency supplies shall be required: One working battery-operated radio in each building used by children.

The battery-operated radio was not working during the inspection.
Plan of Correction: The program director stated that she was able to get the radio to work later on the date of inspection. A back-up radio will be obtained for future use.

Staff records shall contain all required elements.

There was no job title listed in the record for staff 1, 2, 3 and 4.
