Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis screening which shall be submitted at the time of employment and prior to coming into contact with children.
There was no TB screening available for staff 1 who was caring for children during the inspection. Staff 1 has been employed for more than 30 days.
Plan of Correction: The staff member has completed the TB test with a negative reading. The document is now included in the staff file.
Standard 8VAC20-780-510-G
Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.
One prescription medication, for child 1, lacked any type of name label. Additionally the prescription label was not attached with dosing information and times to be given.
Plan of Correction: The student prescription label with child's name, medication, dosage amounts, and time to be given are now located on the student's medication. Evidence of this was sent to inspector on 1/23/26.

Staff records shall contain all required elements.
The record for staff 1 lacked documentation of completion of orientation training required by 8VAC20-780-240.
