During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on February 10, 2025, and completed on February 10, 2025, as a part of the conditional licensure period. The on-site inspection began at 12:30 pm and ended at 1:15 pm. The inspector reviewed compliance in the areas listed above. There were four children present and two caregivers; the point total was eight. The inspector reviewed four children?s records and two caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations

Standard 8VAC20-790-170-A
Providers must obtain a current tuberculosis screening from a caregiver at the time of hire and prior to coming into contact with children. Documentation of a tuberculosis screening for Caregiver 1, who has been caring for children for five months, could not be located.
Plan of Correction: Per the provider: I will have one done.
Standard 8VAC20-790-180-A
A tuberculosis (TB) screening is required every two years from the last screening. An updated TB screening for Caregiver #2 was not completed and was overdue by eight days.
Plan of Correction: Per the provider: I will have one done.
Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There was no physical exam record available for Child #2, who had been in care for five months.
Plan of Correction: Per the provider: I will have the parent provide one.
Standard 8VAC20-800-60-B
The provider shall maintain a child?s record that contains all required information. The record of child #1, enrolled for five months, did not contain the address of one designated person to contact in case of an emergency if the parent cannot be reached. The record of child #2, enrolled for three months, did not contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.
Plan of Correction: Per the provider: I will print out a student record and have them complete the missing information.
