During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on June 16, 2025, and completed on June 16, 2025, as a part of the licensure period. The on-site inspection began at 10:40 am and ended at 12:00 pm. The inspector reviewed compliance in the areas listed above. There were five children present and two caregivers; the point total was eight. The inspector reviewed four children?s records and two caregiver/household member(s) records on site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-800-60-A
The provider shall maintain an up-to-date record at the family day home for each enrolled child. Child #4, in attendance on 06/16/2025, did not have a record.
Plan of Correction: Per the provider: I will contact parents and have them complete all the missing information.
Standard 8VAC20-800-60-B
A child's record shall contain all required information. 1. The record of child #1 did not contain the policy number of the child's medical insurance, proof of the child's age and identity and information on allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency situation. 2. The record of child #2 did not contain the telephone number of each custodial parent's place of employment and the policy number of the child's medical insurance. 3. The record of child #3 did not contain the telephone number of each custodial parent's place of employment, written authorization for emergency medical care should an emergency occur, and the parent cannot be located immediately and the address for one designated person to contact in case of an emergency if the parent cannot be reached.

Standard 8VAC20-800-70-B
Before the child's first day of attendance, the provider shall obtain the parent's written acknowledgement of the receipt of the information for parents regarding family day home operations and administration. The records of child #1, child #2, child #3, and child#4, all who have been in care for five days, did not contain the parents? written acknowledgement of the receipt of information.
Plan of Correction: Per the provider: I will contact parents and have them complete all the missing information.
Standard 8VAC20-800-800-B
The provider shall review the emergency plan at least annually and update the plan as needed. The provider shall document in writing each review and update to the emergency plan. The most recent review of the emergency plan was dated 05/20/2024, exceeding the annual requirement.

Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized. 1. The record of child #2, enrolled for five days, did not contain documentation of immunizations. 2. The record of child #4, enrolled for five days, did not did not contain documentation of immunizations.
