During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on May 22, 2025. The on-site inspection began at 10:15am and ended at 11:56am. The inspector reviewed compliance in the areas listed above. There were 4 children present and 2 caregivers; the point total was 13. The inspector reviewed 4 children?s records and 4 caregiver/household member records on site. This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
3There was no physical exam record available for Child #3, who had been in care for four months.
The records for Child #1 and Child #2, enrolled for over one week and Child #3, enrolled for four months, were missing name, address and telephone number for one designated person. The records for Child #1 and Child #2 were missing the address for one designated person.
The changing pad had a tear and was not considered nonabsorbent and washable.
