An announced initial inspection was conducted between 9:30 A.M.-11:30 A.M. on 06/20/2024 as a result of a planned change of ownership. The licensing inspector conducted a tour of the physical plant, verified floor and site plans, and reviewed the background check findings for the applicant/agents and the program director. Policies and procedures submitted with the initial application were reviewed prior to the inspection. Areas of non-compliance are identified in the Violation Notice.
