During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 07/08/2025, as part of the licensure period.

The on-site inspection began at 9:30 am and ended at 10:30 am. The inspector reviewed compliance in the areas listed above.
There were 4 children present and 2 caregivers; the point total was 11. The inspector reviewed 3 children?s records and 2 caregiver records on-site.

This inspection included document review and tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within 5 business days from today, which will be the close of business on 07/24/2025. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Caregiver #2, who had been employed for three weeks, did not have a completed national criminal background check.
Plan of Correction: The criminal background check was sent a week and a half before caregivers start of employment and took longer than expected. it has already sent to supervisor.
Standard 8VAC20-800-280
Poisonous materials shall be stored in an area inaccessible to children.

Poisonous cleaning chemicals were located in the laundry room with an unlocked door.
Plan of Correction: A lock will be added to the laundry room to ensure and prevent any accidental openings and prevent any chance of any children to get their hands on anything that could cause them harm.

When a high chair is occupied by a child, a safety strap shall be used and securely fastened.

Child #3 was sitting in a high chair but their safety strap was not fastened.
Plan of Correction: Will have to do more training for it to become an important habit to fasten the safety straps for all caregivers.

The provider shall review the emergency plan at least annually and document in writing each review.

An update to the emergency plan was overdue by eighteen months.
Plan of Correction: The emergency plan will be reviewed quarterly to make sure if there in any changes that need to be made and the form for the emergency plan will be updated alike it has to

Emergency evacuation procedures shall be practiced monthly.

Emergency evacuation procedures were not practiced in the months of March 2025, April 2025 and June 2025.
Plan of Correction: The emergency evacuations were noted in a different form and will be moved to the proper form that needed to be filled in the first place.

The menu for the current one-week period shall be dated.

The menu posted was not dated for the current week but for the weeks of 06/02/25 and 06/30/25.
Plan of Correction: The menu dates have will be properly filled from the missing dated and will be filled correctly.
Standard 8VAC20-800-960-B
Infants who cannot hold their own bottles shall be picked up and held for bottle feeding. Bottles shall not be propped.

Child #1 was observed in a swing chair with a bottle propped with blankets.
Plan of Correction: All caregivers will give full attention to the infant when giving them their milk so it doesn't have to be propped on any blankets.
