During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on October 21, 2024, and completed on October 23, 2024, as a part of the conditional licensure period. The on-site inspection began at 11:30 am and ended at 2:45pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and four staff. The inspector reviewed five children?s records and four staff records electronically on October 23, 2024. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on November 4, 2024. A POC submitted after this date will not appear on the public website.
Violations
7The record of child #5 contain documentation of immunizations dated five days after the first day of attendance.
The record of staff #1 contained a tuberculosis screening dated five days after the date of employment.
Four lunch boxes were not labeled with the date.
Child #9 was observed walking around the classroom with a slice of pizza.
Four children were observed in high chairs without the protective belt being fastened. One child was standing in the seat of the high chair.
1. The record of staff #1 did not have documentation that two references were checked prior to employment.
2. The record of staff #4 did not have documentation of a second reference.
