Inspector Notes
An announced, on-site monitoring inspection was initiated on January 27, 2025, and completed on January 27, 2025, as a part of a request to modify the age range of the license. The Department received a written request to modify the age range of the license on January 27, 2025. The on-site inspection began at 1:00 pm and ended at 1:40 pm. This inspection included document review, tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
