An unannounced inspection was initiated on 08/07/2025 and completed on 08/07/2025 in response to a complaint received by the licensing office on 07/30/2025 relating to allegations of supervision and physical plant. The inspector reviewed one staff record on-site. This inspection included document review, tour of the facility, and interviews.

The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
