Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician designee, or an official of the local health department. The record of staff #4: contained a tuberculosis screening dated seven weeks prior to employment.

When centers choose to provide meals or snacks, children three years of age or younger may not be offered foods that are considered to be potential choking hazards. A child, three years of age, was observed being served a whole hotdog on a bun. Staff did not cut the hotdog into bite size pieces until the inspector asked them to do so.
Plan of Correction: Staff cut the hotdog into bite size pieces. Going forward all food that is a choking hazard will be cut up.

When a child is placed in an infant seat or highchair, the protective belt shall be fastened securely. Four children, ranging in age from 12 months to two years, were observed unbuckled in highchairs.

Staff who work directly with children shall, in addition to preservice and orientation training, attend at least 16 hours of training and staff development activities, to include the department's health and safety update course. Training shall be related to child safety, child development, the function of the center, and any required department sponsored training. The record of staff #3 did not contain documentation of completing the department?s health and safety update course for the year 2025. Administration stated the training had not been completed.
Plan of Correction: Per the director: I will make sure training is completed.
