Inspector Notes
An unannounced, on-site renewal inspection was completed on 11/10/2025, as a part of the licensure period. The on-site inspection began at 11:30 am and ended at 12:25 pm The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregiver. The inspector reviewed 3 children?s records and 4 caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1The record for child A is missing an authorization for emergency medical treatment signed by parent. The record for child B is missing authorization for emergency medical treatment signed by parent.
