The required ratio for children aged zero to 16 months is one staff member to four children.
Staff and administration confirmed that on 07/17/24 from approximately 8:50am-9:00am there were 5 infants in care with only 1 staff member present.

Plan of Correction: Due to staff shortages, throughout the center, children were in the process of being moved to a different room to maintain ratios.

Children less than 10 years of age shall always be within actual sight and sound supervision of staff.
Video and administration confirmed that on 07/08/24 at approximately 4:45pm child #1, 3 years old, was left in the classroom unsupervised when staff #1 and the rest of the children exited the classroom and went to another roomin the center. Approximately 15 minutes later another child's parent entered the classroom and found child #1 alone. Staff #1 did not know that child #1 was not present in the room with the class.
The classroom has a back door that exits out onto the front of the building and parking lot.

Plan of Correction: The staff member was immediately suspended, later terminated, and appropriate agencies were notified. All staff were also reminded of the policies regarding active supervision.
