Inspector Notes
An unannounced monitoring inspection was initiated on 10/10/24 and completed on 10/10/24 with a local agency, in response to a self-report received by the licensing office on 10/03/24 relating to supervision. This inspection included document review, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Appropriate supervision of all children in care, including daily intake and dismissal procedures to ensure safety of children.
On 10/03/2024 the center self-reported that a 4 year old child had been left unsupervised on the playground by staff #1, at 3:39pm, for no more than 15 seconds. The child was seen on the playground by staff #4, who was sitting in his car. Staff #4 immediately went to the playground where the child was coming out of the tunnel of the climber crying. Staff #4 brought the child back inside to the front desk.

Plan of Correction: Administration notified licensing to self report the incident. Administration did a full internal investigation and terminated staff #1. All staff were reminded of the requirements, and policies and procedure of supervision.
