Inspector Notes

An unannounced inspection was initiated on 4/28/25 and completed on 4/28/25 in response to a complaint received by the licensing office on 4/25/25 relating to allegations of ratios not being met as required.
This inspection included tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 05/06/25. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.031-A-3-b
The ratio of one staff member to five children from ages 16 months to 24 months is required to be met at all times.

Interviews with staff on 04/28/25 confirmed that on 4/24/25 there were a total of 10 children with only 1 staff from approximately 8am-9:30am. One 4/24/25 an injury did occur during this time where a child fell resulting in a busted lip.

Also, staff interviews confirmed that most mornings, for approximately 30 minutes, ratios have not been met over the last 2 weeks.

The ratio of one staff member to eight children from ages 24 months to 36 months is required to be met at all times.

Interviews with staff on 04/28/25 confirmed that on 4/24/25 there were a total of 16 children with only 1 staff from approximately 8am-10:30am.

Also, staff interviews confirmed that most mornings, for approximately 30 minutes, ratios have not been met over the last 2 weeks.

The ratio of one staff member to ten children from ages 36 months to 5 years is required to be met at all times.

Interviews with staff on 04/28/25 confirmed that most mornings over the last 2 weeks there are a total of 13 children with only 1 staff for approximately 20 minutes.
