Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/24/2024 and completed on 9/27/2024, as a part of, the conditional licensure period. The on-site inspection began at 3:20 pm and ended at 4:00. The inspector reviewed compliance in the areas listed above. There were 12 children present and 2 staff. The inspector reviewed 5 children?s records on-site and 3 staff records electronically on 9/27/2024. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Medication shall be labeled with the child?s name, the name of the medication, the dosage amount, and the time to be given.

Medication 1, belonging to Child 2, was not labeled with the child?s name, the name of the medication, the dosage amount, or the time to be given.
