An unannounced, on-site 60-day monitoring inspection was initiated on 9/9/2024 and completed on 9/9/2024, as a part of the conditional licensure period. The on-site inspection began at 10:15am and ended at 11:05am. The inspector reviewed compliance in the areas listed above. There were 2 children present and 1 staff. The inspector reviewed 1 children?s records and 1 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
