During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on October 22, 2025, and completed on October 22, 2025, as a part of the licensure period. The on-site inspection began at 10:45am and ended at 12:00pm. The inspector reviewed compliance in the areas 16. The inspector reviewed four children?s records and two caregiver/household member(s) records on-site.
This inspection included document review, a tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 22.1-289.057-A
The family day home is required to submit to VDH and implement a plan to test potable water for lead. The provider did not have evidence of a testing plan submitted to VDH.
Plan of Correction: Per the provider: I will review the information and submit a plan.
Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. 1. The record of child #1, enrolled for four months, did not contain documentation of a physical. 2. The record of child #2 contained documentation of a physical dated six months after enrollment.
Plan of Correction: Per the caregiver: I will request the missing information from the parents.
Standard 8VAC20-800-210-A
In addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.1. The record of caregiver #1 contained 13 hours of annual training from the period of July 24, 2024 through July 23, 2025. 2. The record of caregiver #2 did not have any documentation of annual training during the period of July 24, 2024 through July 23, 2025.
Plan of Correction: Per the provider: we will complete the required training.
Standard 8VAC20-800-60-B
The provider shall maintain an up-to-date child?s record containing all required information. The record of child #2, enrolled for 14 months, did not contain the following information: the telephone number of each custodial parent's place of employment, the name, address and telephone number of one emergency contact, the name and policy number of the child's medical insurance, and the parent's signed acknowledgment of the review of the child's emergency contact information.

Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized. The record of child #2 contained documentation of immunizations dated six months after enrollment.
