During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on February 26, 2025. The on-site inspection began at 10:30am and ended at 1pm. The inspector reviewed compliance in the areas listed above. There were 37 children present and 11 staff. The inspector reviewed 5 children?s records and 5 staff records onsite on February 26, 2025. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Staff #1 worked with children at the center for two weeks prior to the center receiving a completed national criminal background check.
Plan of Correction: The center responded with the following: Staff #1 moved from out of state to Virginia. The out of state background checks for Staff #1 were completed and prior to employment and prior to working with children. Staff #1 lived in Virginia 12 days prior to working at the center with children. Moving forward, all background checks from Virginia will be obtained prior to employment and prior to working with children at the center.
Standard 8VAC20-780-440-J
There shall be at least thirty inches of space between service sides of occupied cribs and other furniture where that space is the walkway for staff to gain access to any occupied crib. An occupied crib in the infant room did not have 30 inches of space between the service side and another crib.
Plan of Correction: The center responded with the following: During the time of the inspection, the unoccupied crib was moved to ensure that there was thirty inches of space. The center will ensure that all cribs that are occupied by infants has thirty inches of space between service sides when occupied.
