During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 01/27/2025. The on-site inspection began at 12:30 p.m. and ended at 2:30 p.m. The inspector reviewed compliance in the areas listed above. There were seven children present and two staff. The inspector reviewed five children?s records and four staff records on-site.
This inspection included:

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 02/11/2025. A POC submitted after this date will not appear on the public website.

3There was not a battery-operated radio available in the facility during the inspection.
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.
The record for Child A, enrolled for three months, was missing the following required components: The address for one of the two required emergency contacts. The record for Child B, enrolled twenty days, was missing the following required components: The address for two emergency contacts.
The center shall maintain and keep at the center a separate record for staff that contains all of the required information.
The record for Staff A, employed for one month, was missing the following required components: 1.) One of the two required references to character, reputation, and competency that are checked prior to employment. 2.) Documentation to demonstrate that the individual possesses the education, certification, and experience required by the job position.
