Children's hands shall be washed with soap and running water after eating snack and staff's hands shall be washed with soap and running water before and after a diaper change.

Plan of Correction: All staff have been immediately retrained on proper handwashing procedures, including washing children's and staff hands with soap and running water after meals and before and after diaper changes. Handwashing procedures are posted at all sinks and diaper-changing areas. The director will monitor daily to ensure consistent compliance.

Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Staff #3 used their hands to open the lid of the foot-operated system during disposal.
Plan of Correction: All staff have been retrained on proper diaper disposal procedures, including using the foot-operated system correctly without hand contact. Proper handwashing is required immediately after diapering. The director will routinely observe diapering procedures to ensure continued compliance.

The center shall have in each staff person's record documentation to demonstrate that the

Plan of Correction: Staff #2 is currently enrolled in required training to meet lead teacher qualifications. Documentation of completed and in-progress training is maintained in the staff file. Until qualifications are fully met, the staff member will not be counted as the lead teacher. The director will ensure all staff roles align with documented qualifications and will review staff files quarterly to maintain compliance.
