Inspector Notes

A subsidy health and safety inspection (SHSI) was initiated and completed on September 9, 2025. The inspection started at 12:46 pm and concluded at 4:00 pm. There were 25 children present and 10 staff. The inspector reviewed 10 children?s records and 5 staff records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 16, 2025.

Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Staff #1, who had been employed for 3 months, did not have a completed national criminal background check until 2 weeks after first day of employment.

Staff#2, who had been employed for 7 months, did not have completed national criminal background check until 3 weeks after first day of employment.

Staff#4, who had been employed for 5 months, did not have completed national criminal background checks until 3 months after first day of employment.
Plan of Correction: Due to stipulations we had been previously operating under with our sister
tribe?s subsidy program we also participate with, we were allowed to have new employees start in a classroom to shadow a veteran employee to meet the children and get routine down. New staff members were never left alone under any circumstances (without a completely vetted staff member present) with the children until all 3 background checks were completed. This was due to the original 4?6-week turnaround of the CRS check for each employee and allowing us to operate under the guarantee (not even for a small break) that no employees would be left alone with any children. Chickahominy?s Little Arrows plan of correction for this violation will be to adjust to the VDOE expectations of not allowing any future employees to begin working (or shadowing) until all 3 checks have been run.
