During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 10/6/2025 and completed on 10/6/2025 in response to a complaint received by the licensing office on 10/2/2025 relating to allegations of Supervision and Administration. The inspector reviewed 1 child?s record and (2) staff records. This inspection included document review, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (date). A POC submitted after this date will not appear on the public website.

When staff are supervising children, they shall always ensure their care, protection, and guidance.
A child received several injuries on 10/1/2025 while staff were engaged in other task in the classroom.
The incident reports read that child #1 fell out of a riding toy car around 10:15 am, leaving a red mark on the child's right cheek.

Around 11:13 am child #1 was standing on a stool in front of the sink, another child pulled on the child and the child lost their balance, hitting their lip on the sink. There was no staff to care for the 18 month child standing on the stool at the sink.

Around 2:39 pm child #1 was sitting at the table with another child who was finishing snack, the other child scratched child #1 on the left cheek. One staff was diapering children and the other staff was picking up the cots from rest time. Neither of the staff were protecting the children as they were engaged in classroom tasks.

Staff did not protect child #1 from ingesting foods that were highlighted on a menu from the parent as a food not to be served. The menu dated 6/27/2025 listed gold fish for an afternoon snack. The parent communication app showed that the child consumed the gold fish for afternoon snack.

The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.
The posted list in the toddler room did not include one child's sensitivity to a food item that was addressed on the child's enrollment form dated 4/7/2025.
