Inspector Notes

An announced, on-site initial inspection was conducted on 8/14/24. The inspection began at 10:00am and ended at 11:50am. The initial inspection was conducted in response to a change of ownership. The inspectors reviewed compliance in the areas listed above. There were no children present during this inspection. The inspectors reviewed 2 staff records on-site. This inspection included:
? document review that included but not limited to 2 staff records, center's accountability log, menu, daily schedule, playground schedule.
? tour of the facility, assessment of the indoor and outdoor space and equipment, including first aid supplies and center vehicles.
? observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/19/24. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
Based on observations, areas and equipment, inside and outside the center were not maintained in a clean, safe and operable condition.
Evidence:
1. Handwashing sinks in Room # 2, Room # 5 and one of two sinks in the preschool bathroom were clogged.
2. Peeling paint was found on the back wall in room # 2.
3. The bottom of the tall white cabinet in Room # 3 was broken with edges posing an injury hazard to children.
4. The rug on the toy shelf in Room # 3 was fraying, with loose and hanging strings.
5. One chair in Room # 4 (School age room) had cracks posing an injury hazard to children.
6. Two hooks at the bottom of the wooden cubbies were broken with sharp edges.
7. The covers for 4 boppy pillows appeared to stained and dirty.
8. a. The net in the middle of one of two rock climbing equipment had tears posing a safety hazard for children. The net is meant to be used by children to climb or sit on; b. The second rock climber had a large crack and was broken with sharp edges, posing an injury hazard to the children.
9. The chains for four swings on the playground were found to be rusty.
10. The play kitchen on the toddler playground was found to be covered in dirt.
11. Cribs in Room 3, chairs in Room # 5, play equipment and furniture in Rooms 1 and 5 had visible dirt and dust, the foot pedal trash can in Room # 3 had stains and visible dirt, two of three vans for transporting children had surfaces under the windows that were covered in dirt and the carpet in one of three vans had large dirt stains.
Plan of Correction: 8/14/24: We acknowledged and put a plan of correction in place to take place immediately. A plumber will be in today to review and fix any issues resolving the sink. The shelf in the infant room has been replaced with a shelf that is in great condition. The hooks at the bottom of the cubbies have been removed and new hooks have been ordered. The cracked chair was disposed off during the initial inspection. New covers for the boppy pillows have been ordered and the existing covers will be replaced. Both rock climbing toys have been removed from our playground area. New chains for the swings will be purchased at Home Depot. any toys with dirt or stains will be deep cleaned. Any toys that remain dirty will be removed. Staff will deep clean the crib surfaces, furniture, all toys and the vans. Staff will also maintain the cleaning to ensure that all equipment is in operable condition.
