Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on July 22, 2025, as a part of the licensure period. The on-site inspection began at 9:15am and ended at 11:35am. The inspector reviewed compliance in the areas listed above. There were 33 children present and 10 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-1
The center is required to obtain a completed sworn statement prior to the employee's first day of employment.

The sworn statements for Staff #3, #4, and #5, all currently working, did not have documentation of each sworn statements. Staff #3, #4, and #5 completed each sworn statements the date of the inspection, one month after the first date of employment.
Plan of Correction: The sworn statements for Staff #3, #4, and #5 were completed the day of the inspection. Moving forward, the director is responsible for using a check list for staff files to ensure all required background checks are obtained prior to the first day of employment.
Standard 8VAC20-780-240-A
Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.

Staff #1, who is working with children, has not completed the VDOE sponsored online orientation course which is overdue by three months.
Plan of Correction: Staff #1 will complete the VDOE sponsored online orientation course. Moving forward, management will ensure staff members complete this required orientation course within 90 days.
Standard 8VAC20-780-280-E
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by the children.

In the two/three year old classroom, two bottles of disinfectant, a bottle of bleach and water, and air freshener were in an unlocked cabinet, out of reach of children.
Plan of Correction: A lock was placed on the cabinet in the two and three year old classroom. Moving forward, staff will be retrained on ensuring all hazardous substances are locked away.

Each restroom area provided for children shall be equipped with soap.

The restroom for the two and three year old classrooms was not equipped with soap.
Plan of Correction: Soap was placed in the bathroom the day of the inspection. Management will walk through the building to ensure all sinks are equipped with soap.

Staff records shall be kept for each staff person with required information.

Staff #4 and Staff #5's records did not contain the name, address and telephone number of a person to be notified in an emergency.
Plan of Correction: Emergency contacts were obtained for Staff #4 and #5 the day of the inspection. Moving forward, management will use a checklist to ensure that all required information for staff records are obtained.
