Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 10/3024, as a part of the conditional licensure period. The on-site inspection began at 9:30 am and ended at 1:20 pm. The inspector reviewed compliance in the areas listed above. There were 25 children present and 12 staff. The inspector reviewed nine children?s records and four staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/15/24. A POC submitted after this date will not appear on the public website.

The sworn statements for Staff #2 & 3, who are currently working, were not dated.
Staff #2 and Staff #3, who had been employed for three months, did not have a central registry search requested.
Staff #1, #2, and #3 had no documentation and had been employed for past three months.
Staff #3, who has been employed three months, did not have information on file showing how program leader qualifications were met. Staff #3 was working as program leader on the day of the inspection.
Video footage on 10/30/24 showed that there was one staff member with one child from 6:02 am ? 6:20 am.
Child #1, medication given to center three months ago; child #2, medication given to center seven months ago; child #6, medication given to center four months ago; child #7, medication given to center one month ago; child #8, medication given to center two months ago; child #9, medication given to center four months ago. There were no written authorizations on file from the parents.
Six non-prescription medications were in the medication box. These medications have been at the center for one to six months with no written authorization available or renewed. There was no written authorization from Child 8?s physician and parent for the long-term prescription medication was in the medication box.
Staff #2, #3, and #4, who have been employed for three months, did not have documentation of education or certification on file.
