Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/3024, as a part of the conditional licensure period. The on-site inspection began at 9:30 am and ended at 1:20 pm. The inspector reviewed compliance in the areas listed above. There were 25 children present and 12 staff. The inspector reviewed nine children?s records and four staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/15/24. A POC submitted after this date will not appear on the public website.

Program leaders shall be at least 18 years of age, have fulfilled a high school program completion or the equivalent.

During the stated hours of operation, there always shall be on the premises when one or more children are present one staff member who meets the qualifications of a program leader or director and an immediately available staff member, volunteer or other employee who is at least 16 years of age.

Prescription and nonprescription medication shall be given to a child, only with written authorization from the parent.

The center?s procedures for administering medication shall include duration of the parent?s authorization for medication, provided that it shall expire or be renewed after 10 workdays. Long-term prescription drug use and over-the-counter medication may be allowed with written authorization form the child?s physician and parent.
Six non-prescription medications were in the medication box. These medications have been at the center for one to six months with no written authorization available or renewed. There was no written authorization from Child 8?s physician and parent for the long-term prescription medication was in the medication box.
