8VAC20-780 Programs.

During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on 12/19/2024 and completed on 1/15/2025 in response to a self-report received by the licensing office on 12/19/2024 relating to behavioral guidance and forbidden actions. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/7/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-400-A
To promote the child?s physical, intellectual, emotional, and social well-being and growth, staff shall interact with the child and one another to provide help, comfort and support.
Staff #1 was seen on video pushing child #1 away several times. The third time resulted in child #1 falling to the floor and crying. Child #1 was not consoled.
Plan of Correction: Staff member was removed from classroom immediately upon notice of this incident, and they no longer work in the facility. We do not tolerate staff behavior that is not comforting or supportive of each child's needs.

The following actions or threats thereof are forbidden:
Roughly handling- Staff #1 was seen on video pushing child #1 away several times. The third time resulted in child #1 falling to the floor and crying. Video footage also showed staff #1 pushing child #2 away by tapping him on his head and then pushing him back causing the child to retreat to the floor.
