Inspector Notes

An unannounced, on-site monitoring inspection was completed on 12/04/2024, as a part of the conditional licensure period. The on-site inspection began at 3:00 pm and ended at 4:50 pm. The inspector reviewed compliance in the areas listed above. There were 30 children present and 4 staff. The inspector reviewed 5 children?s records and 5 staff records on-site on 12/04/2024. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

Several children were play fighting, running, standing on cafeteria tables tools, and roughhousing while waiting to transition from snack to tutoring. Three staff members were at a table working on attendance and one staff member was positioned at the doorway. Staff did not redirect the children's behaviors for approximately 5 mins.
Plan of Correction: Staff will be retrained on child engagement during transitions.
Standard 8VAC20-780-550-D
The center shall implement a monthly practice evacuation drill.

There were no evacuation drills practiced, the center has been operating since the end of September 2024.
Plan of Correction: Staff will ensure monthly evacuation drills are practiced.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries that contains all of the required information.

There was no time, date, method of parental notification, future action to prevent recurrence, and only one of two required signatures on the injury report for child #6 dated 11/15/24.
