Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Staff #2 and Staff #4, who have been employed for two and half months, did not have a completed national background check.
Staff #2 and Staff #4 were never left alone with children and always were supervising children with another staff member that has completed background checks.
Plan of Correction: The provider will follow up with the agency for results for the criminal background checks for Staff #2 and Staff #4. Moving forward, the criminal background results will be received before the date of hire, and if results have not been received, a staff member will follow up with the agency for results.

The provider is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Staff #1, who has been employed for 17 months, did not have documentation of a sex offender registry check and a search of the child abuse and neglect registry from one state.
Staff #2, who has been employed for two and half months, did not have documentation of a criminal history record information check, sex offender registry check, and a search of the child abuse and neglect registry check for one state that the staff member resided in the preceding five years.
Staff #4, who has been employed for two and half months, did not have documentation of a sex offender registry check, and a search of the child abuse and neglect registry from one state that the staff member has resided in the preceding five years.
Plan of Correction: All out of state background checks will be conducted before the first day of employment. Searches will completed for Staff #1, #2 and #4 by 11/21/2025. Moving forward, all out of state background checks will be conducted for all staff who have resided in a state outside the state of Virginia in the preceding five years.
