Inspector Notes

An unannounced, on-site initial inspection was conducted on 7/31/24, as part of the initial licensing process. The on-site inspection began at 10:03am and ended at 1:00pm. The inspectors reviewed compliance in the areas listed above. There were 0 children and 3 staff present. The inspectors reviewed 0 children's records and 8 staff records while on-site. This inspection included document review, tour of the facility, interviews and square footage measurements. Information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.

Based on interview conducted, the center does not have a written emergency preparedness plan. Evidence: During the inspection staff reported that the emergency plan was with the high school and not available for review on this date. Staff agreed to send the plan to the inspector for review by 8/9/24. To-date, 8/13/24, the emergency plan has not been submitted for review.
Plan of Correction: The Emergency Plan is attached.

Based on document reviewed, the emergency preparedness plan did not include procedural components.

Evidence:

1. The plan did not include the establishment of an emergency office and back-up officer to include 24-hour contact telephone number for each.
2. The plan did not include a plan for the continuity of operations to ensure the essential functions are maintained during an emergency.
