Inspector Notes

An announced, on-site initial inspection was conducted on 7/18/2024,as a part of the initial licensure approval. The on-site inspection began at 11:31am and ended at 12:40pm. The inspector reviewed compliance in the areas listed above. There were no children present and the provider. The inspector reviewed 3 staff records on-site on 7/18/24. This inspection included document review of policies and procedures, authorization forms, a tour of the home, interviews,
and observation. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on observation and interview the provider did not have all first aid kit supplies.
Evidence:
1. First aid kit contained one triangular bandage.
2. First aid kit did not contain First Aid instruction manual.
