Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/10/24 and completed on 10/10/24, as a part of the conditional licensure period. The inspector reviewed compliance in the areas listed above. There were 16 children present and 2 staff. The inspector reviewed 4 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-C
During the stated hours of operation, there always shall be on the premises when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, who is at least 16 years of age.

On 10/10/24 from 11:53 am until 12:33pm there was only one (1) staff present with 16 children in care aged 5 years-11 years.
Plan of Correction: There will always be 2 staff present during the hours of operation.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On 10/10/24 a child went down the hall to use the restroom unsupervised. The staff member could not see or hear the child, and there was a door beside the restroom that lead outside to the main parking lot of the school.
Plan of Correction: Children will be supervised at all times as required.
