Standard 8VAC20-780-180-C
Staff who drive a vehicle transporting children shall disclose any traffic violation that occurred prior to or during employment or assignment as a driver.

There was no documentation of driving history in the file of Staff 1, and the staff member has been employed for over 1 year.
Plan of Correction: The staff had clean driving record and kept license copy and insurance.
Standard 8VAC20-780-420-A
Before the child's first day of attending, parents shall be provided in writing the following: the center's transportation policy;

1) The facility's policy stated children are "transported safely" to the child care center, but the facility are putting two children in one safety restraint when transporting to and from the facility.
2) The facility's policy stated that "emergency contact information for each child is always kept in the vehicle during transportation", but emergency contact information was not kept in the vehicle.
3) The facility's policy states that "written parental permission is required for transportation", but there was no documentation of parental authorization for transportation in Child 1-6's record, all of which are transported either to or from the local school.
Plan of Correction: We provide policy during tour/enrollment for pickup/drop off. We make sure parent sign forms. We keep emergency form in vehicle always. There after we will keep authorization letter in vehicle. We will not put children in one seat.
Standard 8VAC20-780-550-K
The center shall prepare a document containing local emergency contact information, potential shelters, hospitals, evacuation routes, etc., that pertain to each site frequently visited or of routes frequently driven by center staff for center business such as field trips or pick up or drop off of children to or from schools etc. This document must be kept in vehicles that centers use to transport children to and from the center.

There was no documentation of emergency contact information, for Child 1-6, kept in the personal vehicle of a staff member, used to transport children.
Plan of Correction: We will keep all emergency contact and shelters in vehicle with document while transport children to and from the center.

The center shall ensure that during transportation of children: Virginia state statutes about safety belts and child restraints are followed as required by ?? 46.2-1095 through 46.2-1100 of the Code of Virginia, and stated maximum number of passengers in a given vehicle is not exceeded;

Staff 1 admitted to the inspectors that she often puts 4 children in the rear bench of her van, which is only designed to safely secure 3 persons. She stated that she has 2 children share a seat and a seatbelt while being transported to and from the school.
Plan of Correction: We will not put exceed passengers and make sure each child put seat belt while transportation. Right away.

Parental permission for transportation and field trips shall be secured before the scheduled activity.

Child 1-6 did not have documentation of parental permission for transportation to and from the school. The children hare school-agers and have been in care anywhere between 4.5 months and 16 months.
Plan of Correction: We keep emergency contact info with all phone numbers in the vehicle and parent signed. There after we add authorization form.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: first day of attendance, physician contact information, emergency contact information.

1) Child 4, 5 and 6 had records that were kept all together and not separate.
2) Child 4, 5 and 6 did not have a second emergency contact, and the first emergency contact did not have an address.
3) Child 1 had incomplete emergency contact information (no addresses) for either emergency contact.
Plan of Correction: We keep children each separate record and make sure parents fill all contact information. We make sure parents filled same day of enrollment.
