During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 4/22/2025. The on-site inspection began at 11:25 AM and ended at 1:15 PM. The inspector reviewed compliance in the areas listed above. There were 30 children present and 6 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.011-F
Documents required for the Superintendent shall be posted in a conspicuous place on the licensed premises.

The most recent inspection of the facility (2/4/2025) was not posted on the premises.
Plan of Correction: I will post the report for today's inspection.
Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.

Child #5 (11 months) most recent immunizations are dated from when the child was 4 months old.
Plan of Correction: We will contact the family and have them provide an updated copy of immunizations.
Standard 8VAC20-780-440-B
Cribs, cots, rest mats, and beds shall be identified for use by a specific child.

The cots in the Preppers and the cots in the Toddler A classroom were not identified for use by each specific child.
Plan of Correction: The cots were labeled with a number, however the cot chart did not identify which child coordinated with each number. We have a staff meeting tonight and the staff will be reminded to update their cot charts.
