During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/18/2025. The on-site inspection began at 10:40 AM and ended at 1:50 PM. The inspector reviewed compliance in the areas listed above. There were 45 children present and 13 staff. The inspector reviewed 6 children?s records and 8 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-350-B-2
The staff-to-children ratio of 1:5 required for children aged 16 months up to 24 months.

The youngest child in the Toddler B classroom is 19 months old. During the inspection, there was 1 teacher observed supervising 6 children alone.
Plan of Correction: The second staff member was in the kitchen. They will be written up for violating policy. Any staff member that leaves a classroom out of ratio will be written up.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.

The medication authorization for child #6s emergency medication was dated 7/31/2025 and expired on 8/14/2025. The medication was observed on-site at the facility and was not returned to the family or disposed of.
Plan of Correction: We will start using the state model form so that parents know that the authorization must be signed by a doctor for long-term administration. I will have the family sign a new short term authorization until a doctors authorization is obtained.
