During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 3/11/2026. The on-site inspection began at 11:40 AM and ended at 1:45 PM. The inspector reviewed compliance in the areas listed above. There were 60 children present and 11 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
51. Child #2's (date of enrollment: 6/2/2025) record does not contain documentation of a physical exam.
2. Child #4's (date of enrollment: 8/18/2025) record does not contain documentation of a physical exam.
In the Toddler B classroom, child #6 was observed sipping from a sippy cup. The staff in the classroom recognized that the sippy cup did not belong to child #6. The staff person took the cup from child #6 and placed it in front of child #7 who was still eating lunch. The sippy cup was not cleaned, sanitized or put away to be returned home to child #7.
A child was observed getting their diaper changed in the Toddler B classroom. The child was returned to the floor and laid down on their nap mat to rest after the diaper change. The child did not have their hands washed and the staff member did not wash their hands after the diaper change.
A child was observed having their diaper changed in Toddler B. The changing table was not cleaned or sanitized before the next child was placed on the changing table for a diaper change.
During lunch, the staff in Infant B and Toddler a were observed setting out nap mats and cleaning the classroom. The staff did not sit with the children.
