During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 08/13/2025. The on-site inspection began at 11:30 a.m. and ended at 12:45 p.m. The inspector reviewed compliance in the areas listed above. There were ten children present , three household member children and two caregivers; the point total was 29. The inspector reviewed seven children?s records and two caregiver/household members records.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/21/2025. A POC submitted after this date will not appear on the public website.

Caregivers required to have Medication Administration Training (MAT) are required to be retrained every three years.

The provider is required to have MAT training based on their policy and due to accepting a prescription emergency medication for a child. The provider's MAT training expired more than six months ago.
Plan of Correction: MAT training is scheduled for renewal.

Items tied across the top or corner of a crib shall be removed when the child begins to push up on hands and knees or is five months of age, whichever occurs first.

A sound machine was observed attached to the top rail of the crib observed in the nap room for child A, age 13 months.
Plan of Correction: The sound machine has been removed from the crib.

Soiled disposable diapers and wipes shall be disposed of in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal.

The foot operated storage system was in the shower of the bathroom, not accessible by the changing table. The provider removed the soiled diaper from the changing table, walked it to the bathroom and used their hand to open the lid to dispose of the soiled diaper.
Plan of Correction: The diaper pail has been replaced with a foot-operated trash can.

Medication shall be kept in a locked place using a safe locking method that prevents access by children.

An emergency medication was observed in an unlocked cabinet located above the counter of the food-prep area where child care is provided.
Plan of Correction: The medication cabinet is now secured with an external lock.
