Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 4/6/2026 and completed on 4/8/2026, as a part of the licensure period. The on-site inspection began at 10:32AM and ended at 11:25AM. The inspector reviewed compliance in the areas listed above. There were 6 children present and 3 staff. The inspector reviewed 4 children?s records on site on 4/6/2026 and 3 staff records electronically on 4/6/2026. This inspection included - document review, tour of the facility, and observations.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center must request a search of the central registry prior to the employee's first day of employment. Staff #2 and Staff #3 (employed for over two years) did not have a central registry search requested.

REPEAT VIOLATION
Sworn statement shall be completed prior to the first day of employment. Staff #2 (employed for over 2 years) did not have documentation of a completed sworn statement. This was previously cited on 8/13/2025.
